if you lost or damage your BSNL SIM card, then you can get back that in charges of Rs. 100 from their Customer Service centre. You have to also submit your Address proof photocopy and your photograph with it and fill form as per required norms.
100 rupees applicable only on 2G prepaid and Postpaid SIM, for 3G you have to pay 139 indian rupees. It is happen with many persons, may be if you are a customer of other telecom operators like Docomo, Aircel, Airtel, Reliance then you can apply the same procedure to get back your Lost SIM Card.
But the charges for replacement or taking back the old number on New SIM card are different as per service terms and conditions. You better confirm from your customer care or service centre first.
